The psychopathic Jigsaw (Tobin Bell) is slowly dying of a brain tumour. He has kidnapped a doctor to keep him alive long enough to play one last game. A grieving father faces impossible choices as he confronts the people responsible for his son's death.

Film Trivia

• The producers of this film asked the producers of Scary Movie 4 if they could use their bathroom set for this movie as it was an exact replica of the sets used in Saw and Saw II.
• Tobin Bell donated two vials of blood to be mixed in with red ink for the printing of 1000 promotional posters.
• Donnie Wahlberg reprises his role as Detective Matthews for a few scenes in this movie.
• The final draft of this script was written in under a week by Leigh Whannell.
